A Patient-Centered Medical Home (PCMH) is not a physical place for you to get care – it is way of giving care that is about a partnership between you, your personal doctor, and your care team. At a PCMH practice, you build a trusting, long-term relationship with your doctor and your care team, and you work together to make sure that you get the care you need to stay healthy.

A New Way for Better Care

At a PCMH practice, you work with a dedicated care team (your doctor and other staff who work at the clinic to keep you healthy) to create a personalized care plan to help you meet your health goals. Whether you need extra help with managing a chronic condition like diabetes, want to lose weight, or want to quit smoking, you can depend on the support of your care team.

Your PCMH care team can help you:

  • Understand and manage your medications
  • Arrange for any tests or treatments that you may need
  • Coordinate your visits to specialists
  • Find useful resources, like support groups or health classes
  • Get answers to your health questions when you need them. Many of our providers have evening and weekend hours, offer same-day appointments, and are available to answer your questions by phone or secure message.
