Delivering high-quality care management for diabetic patients, this program features regular education classes, support groups, and technology to support care management in the home. The program provides patients with special equipment to measure blood sugar, weight, and blood pressure–information that is subsequently sent electronically for a nurse to read. The nurse follows up one or more times a week to discuss the readings, design weekly meal plans, and develop strategies to help control weight, blood sugar, and blood pressure.
Neurologists and nurse educators work closely with patients and their families to make living with the disease more manageable through intensive physical and occupational therapy, onsite counseling, and support groups.
