Diabetes Care

Diabetes is a serious health threat which is rapidly approaching epidemic proportions, particularly in our community. We aim to form a partnership with our patients to bring their potentially debilitating disease under control. Our goal is to improve patient outcomes by providing one-stop care through a focused diabetes session in primary care medicine, as well as in our endocrinology specialty service.

NYC Health + Hospitals/Metropolitan is accredited as a Diabetes Center of Excellence for providing high-quality, evidence-based education to patients with diabetes. The prestigious four-year accreditation from the Association of Diabetes Care and Education Specialists verifies that the hospital’s program meets national standards for diabetes self-management education, which have been shown to be cost-effective and improve clinical outcomes and patients’ quality of life. The standards highlight the significance of ongoing support for people with diabetes, particularly to encourage behavior change and to address psychosocial concerns. We provide individual counseling, as well as group classes, which are currently held virtually in light of COVID19. We normally offer weekly Diabetes Support Groups and monthly Cooking Classes, however these programs are on hold for now until it is safe to gather in groups once again.

Individual patient progress is tracked over time, with specific focus on indicators such as blood glucose, blood pressure, cholesterol, weight and body mass index, and depression screening. Our team of diabetes specialists pay particular attention to some of the most debilitating side effects of uncontrolled diabetes, including loss of sight, lower extremity amputations, and cardiovascular damage.

Patients are part of a comprehensive care team that includes physicians, certified diabetes educators and dietitians, nurses, a social worker, and a patient care associate. Our certified diabetes educator facilitates regularly scheduled telehealth-based group classes in both English and Spanish. Patients can also be referred to the endocrinologist for diabetes management. Foot and eye specialists provide regular examinations. We now have a retinal screening room designated in the Adult Primary Care practice specifically for our Diabetic patients.

Our weekly diabetes drop-in support group is currently on hold due to COVID. We look forward to resuming the group once drop-in group once COVID restrictions allow.
