Primary & Specialty Outpatient Care

NYC Health + Hospitals/Metropolitan strives to create a truly patient-centered environment where patients receive the best individualized care. Our extensive Department of Medicine offers adults over 18 years old a wide range of high quality primary and specialty medical services to ensure patient satisfaction and optimal health.

Metropolitan’s pediatrics, adult primary care, and HIV primary care services have been awarded Recognition by the National Committee for Quality Assurance Physician Practice Connections® – Patient Centered Medical Home™ Program. The Patient Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement. The medical home designation recognizes Metropolitan’s model of care, where each patient has an ongoing relationship with a personal physician who leads a team that takes collective responsibility for comprehensive care of the patient. As a medical home, we ensure that coordinated care and long-term healing replace episodic care based on illness.
