Prepare For Your Appointment

When you first visit the doctor, we will review your medical history, examine you, order any necessary tests or provide referrals to other services you may need. To help get the most out of this visit, please bring:

• All your medication bottles
• Name, address, and phone number of your pharmacy
• Record of immunizations/ vaccinations
• List of medical equipment you use, e.g. glucometer
• Questions you want to ask the doctor
• Your insurance card if you have one

COVID-19 tests are available FREE at all NYC Health + Hospitals testing sites. Insurance is not required to receive a free COVID-19 test or an antibody test at any NYC Health + Hospitals location. If patients have insurance, we bill the insurer directly and never charge a co-pay or co-insurance.

The charges listed below reflect the standard charge amounts that the government requires us to publicly post and do not reflect costs to patients.
